2. Visit our Policies & Forms page for instructions on how to obtain a USA Diving Membership. Read our "Class Information and Payment Policies" form. Download, print, and fill out our "Iowa Diving Club Release and Waiver" form and our "Iowa Diving Club Medical Release" form.

 

3. Bring the two completed forms to your diver's first class.
